After you submit this form, you will hear from us within 48hours to acknowledge the reservation and request payment. Once payment is received we will contact you via email confirming the booking. Medical Consent

This section is to be completed by parent or guardian of participant. I allow my child to participate in the Swimming Academy Clinic sessions. I consider my child to be physically fit and capable of full participation, but in the event that he/she should be injured when I am not present, I give my permission for the teacher/coach to obtain emergency medical and/or dental treatment on his/her behalf.

Programme Changes:

Cranleigh School Swim Academy reserves the right to cancel a course if there are insufficient numbers on either a course or within any age category. In such circumstances your money will be refunded.

Cancellation:

In the event of You cancelling, a refund will only be given if the space can be filled by another participant. If the space is not filled you will incur the full cost of the missed clinic.

Incompatibility:

Cranleigh School Swim Academy reserves the right to exclude or refuse entry to the Lesson if it believes that any person is causing damage to the facilities, or is disruptive to the other attendees. No refund will be given under these circumstances.

Medical Details & Emergencies:

It is imperative that full details of all medical are declared on the Booking Form for each participant. In the event of a medical emergency we ask that all parents stay on site for the duration of each clinic. The Academy should be made aware of all drugs and other medication carried with the participant, parents/guardians ensuring that all items are clearly marked.

Liability:

The Academy accepts all responsibility for those elements of the course under its direct control if caused by proven negligence of the Academy or its employees including death or personal injury. The Academy does not accept responsibility or liability for the loss of enjoyment arising from but not limited to the following: travel arrangements, strikes, acts of terrorism, bad weather, loss or damage to personal property, personal injury or illness suffered whilst on the Course.

It is a legal requirement that we request your permissions to photograph / video your child. We may wish to film your child swimming in order to play back to them later for analysis. These videos will not be shared outside of the session and will be deleted at the end of the course of clinics.

Several times a year Cranleigh School Sports Centre will contact you with relevant information regarding upcoming clinics / workshops. We would also like to be able to be able to contact you about new facilities, courses and opportunities that we think may be of interest to you.
